does anybody know where to buy these pcb boards and what type and spec of relays to use?

Here is an easy, but expensive, way to go if you don't want to source & build them:

Indeed. These are so good and so reasonably priced I can't rationalize messing with other solutions.

I am late to the party
Just wanted to say that I have a separate relay power supply board and individual relay boards.

You can use a relay socket to plug in relays or solder the relay direct to the board. You can swage turret lugs onto the boards so you can do wire wraps.

I use Doug's relays in both of my amps. They were easy to hook up and work great.

There is also quite a bit of info on Hoffman's forum with schematics, layouts and pictures showing how to hook up the relays, spdt switches, foot switch jack, etc.....
