As a quick check I pulled up the schematic, attaching here. This amp is a pretty complex amp (in sold state terms) so debugging it is not a very simple task. It also sounds like you may have done more common techniques for non technical people to check a few known problems. If you're not familiar with opening up tube amplifiers, draining the caps and ensuring you're safe inside them, I highly recommend you stop where you are, and take it to a qualified technician. Otherwise the amp can kill you.

With that said, to really be able to do much more, we'd likely need photo's of the inside. This type of problem can be one of many things. Any of the preamp tubes may be bad, or things across the entire amp may be wrong. if the power supply is putting out way too low voltages for some reason, then you may get this kind of low output across the entire amp. There might be a problem with the heater winding, there may be problems with resistors or capacitors in the flow of the signal...

That's the first quick things that come to mind and I'm sure I'm missing 100 others.

If you DO know how to work with extremely high voltage DC, and have a signal generator and an oscilloscope, you can trace the signal you input with a signal generator through the amp to find out where it's dropping out...

There are some very easy things to do by just looking at the amp, you might see burnt components etc.

If you feel safe enough to open the amp and not hurt/kill yourself and can drain the main filter caps safely, take some photos of the inside of the amp and post them so we can review. If all of this seems complex, take it to an amp tech and be done with it...

Back in 1997 I posted a tube amp debugging guide for help with problems like this. It's here: http://www.geofex.com/ampdbug/ampdebug.htm

It doesn't get all problems, obviously, but it's a good place to start, and quickly leads to the most likely solutions. Using the process and it NOT giving a good solution is a valid clue, too, as it eliminates a lot of not-useful guesses.
