Phil, the first occurrence was actually on a different amplifier and a few years ago. I made it sound confusing by referencing that incident inside the description of what happened with this amp, but if you check the OP you'll see that I mention I saw it happen before and the amp played fine for years afterwards without any sign of stress or failure (under prolonged cranked conditions, too) who knows, that amp (OT specifically) may be hanging by a thread, but it's working fine :)

My apologies for the confusing wording/reference.
