Power transformer fried... why?

Could be just a bad unit. Sometimes all it takes is a nick in a bit of insulation, or something silly like that. In 10 years, we've changed about 5 or 6 transformers, and most of them were failures we could directly attribute to something: oversized fuse, running an amp without a speaker, or a manufacturing issue. If the replacement was wired in and things work fine, then perhaps it was just a random failure.
