D'Lite - where to have a fswtch installed

Brown Note D'Lite 22 - I want to have a 2 button footswitch installed to toggle the OD and/or PAB. Where can I get the work done in Chicago, and what will I need to bring them? I bought the amp used and don't have the schematics.

Here's a layout with and without the relays. It's just not a simple jack installation; if it's not a turnkey install then you will need to supply the relay boards, footswitch and maybe a chassis jack.
