you are also missing the 100pf cap in the vol pot.  Form the top to the wiper.  I think some one suggested going back to the Schems and layout posted on the file sections...Those are right on the money you should take a look at them.  On the pre amp board where you have the B+ coming in is the top the ground for the 220Ks?

My thoughts at this point -- 
The amp did replicate the guitar sound for a little while - without distortion. So, this seems to indicate that the problem is between the first and second preamp stages. So far I've only tried switching the coupling cap with no success, so I can rule that out.
I've recycled the power supply resistors so many times that it seems obvious that they've finally crapped the bed. Can anyone see how this would cause the problem I'm having?
My speaker cable *might* (unlikely, I've used it before) be bootleg; though again I can't see how this would cook the output tubes while leaving the OT cool.
Could I have fried the output tubes by not grounding the 220k resistors and now I have tube problems?

I think that is very possible. With no Grid  bias , destructive runaway is possible.
